- A statement under penalty of perjury that you have a good faith belief the material was removed due to mistake or misidentification
- Your name, address, telephone number, email, and consent to the jurisdiction of a federal court
- A statement that you will accept service of process from the person who submitted the original DMCA notice
4. Repeat Infringers
We will terminate accounts and access of repeat infringers in appropriate circumstances.
